Live Well Communities was launched in 2016 focusing on historically underserved areas of the county - beginning in the communities of Southeastern San Diego, as well as nearby locations in Lemon Grove, Spring Valley, and National City.

The goal of Live Well Communities is to address long-standing inequities, disparities, and disproportionality in this geographic area, by focusing on key interventions that will engage the community, strengthen existing services and serve for improved results.

During the planning stages of Live Well Communities, a nation-wide challenge was announced by the Aetna Foundation, the American Public Health Association and the National Association of Counties. This Challenge offered the opportunity to communicate our efforts on a national platform and to learn from other counties striving for improved wellness.

Plans for a new County Live Well Center are in the works. The newest Live Well Center will be in Southeastern San Diego and when it is finished, the 80,000-square-foot center will offer numerous health and social services under one roof.

Community Mentorship Summit Program
The first annual Community Mentor Summit 2018 brought together individuals who represent the mentoring movement: grassroots mentors with lived experience in the juvenile and criminal justice system, key juvenile justice leaders, government and civic leaders. This conference featured the long-standing work and legitimacy of community mentors with lived experience – past, present and future.

Live Well Communities Listening Sessions - Moderator Guide and Summary
A total of 5 listening sessions were held with community residents within the catchment area between February and June 2017. Sessions were inclusive and reflective of community diversity, with participation from service providers, various age groups, and ethnic groups, including native Spanish-speakers.

Mental Health Town Hall - Summary
Mental Health Town Hall Meeting was hosted by the County of San Diego in participation with a local ethnic newspaper. The meeting intent was to engage the Southeastern San Diego community in a discussion about mental health and implementation of services at the future Southeastern Live Well Center, slated to open in 2021.

Community S.O.A.R Analysis - Summary
On May 4, 2016, community leaders, organizations, and individuals convened and completed a Strengths, Opportunities, Aspirations, and Results (SOAR) analysis for multiple areas of influence.

Resident Leaders In Action - Flyer
Resident Leaders in Action (RLIA) program built upon the existing RLA structure and engaged six previous Resident Leadership Academy graduates as participants in an extended “Train the Trainer” module.

Strong Families, Thriving Communities

The Clinton Health Matters Initiative, the County of San Diego, and the San Diego Foundation have partnered to improve the health and wellness of children and families who interact with San Diego County’s child welfare and juvenile justice agencies.

Lemon Grove Heal Zone

The Lemon Grove HEAL Zone is a community collaborative funded by Kaiser Permanente and convened by Community Health Improvement Partners (CHIP). This 3-year, 9-month program will empower residents to lead healthier lives through environmental changes that are sustained by policies and enhanced by education and promotion.
